If you do not have an attorney for your divorce case, the lists below are the basic forms you will need to file for divorce or to respond (answer) if a divorce has been filed against you. These are likely NOT the only forms you will need to file. After you file the basic forms listed below, your local court may require you to file some additional forms.

TO FILE A DIVORCE COMPLAINT WITHOUT CHILDREN

You will need to complete and file these forms to start your case:

  • Complaint for Divorce Without Children
  • Affidavit of Income and Expenses
  • Affidavit of Property
  • Request for Service
  • Motion and Affidavit for Temporary Orders (Optional)

TO FILE A DIVORCE COMPLAINT WITH CHILDREN

You will need to complete and file these forms to start your case:

  • Complaint for Divorce With Children
  • Parenting Proceeding Affidavit
  • Affidavit of Income and Expenses
  • Affidavit of Property 
  • Health Insurance Affidavit
  • Request for Service
  • Motion and Affidavit for Temporary Orders (Optional)

TO RESPOND TO A DIVORCE COMPLAINT WITHOUT CHILDREN

You will need to complete and file these forms to respond to a divorce complaint:
